When I try to create a Family Account on the Fitbit app, I am unable to do so. The app freezes and remains as pictured below:
Additionally, I've noticed that this problem only affects iPhone users. If you use an Android device—or any other non-iOS product—it works and allows you to set up a family account.
Please advice.

My understanding is the Fitbit Family Account has been set to End-of-Life as a service. In other words, they are not adding any more Family Accounts and continuing to support existing ones until they "sunset" (i.e. shutdown) the service altogether. This is due to the Google purchase of and subsequent integration with Fitbit.
From here and into the future, new users will be required to use their Google account, and this will be the authentication method of choice for use the service. There's precedence for this with the purchase of Nest by Google some years back.
To point, you no longer have the option to create a new account on the web portal:
Unfortunately, this has created an issue for anyone looking to setup an ACE-series device (restricted to use by children by Fitbit) or use their Supervised Google Account (accounts intended for children 13 and younger). The Supervised Google Account is managed by the Google Family Link service where a parent can create or "adopt" an existing account for their child. This is fairly straightforward to do, but the problem lies with the integration. Currently, the Fitbit app is restricted by Google and may not be used with Supervised Accounts.
That said: there is a possibility they're in the process of addressing this issue. I was recently directed by Fitbit Support to create a new Supervised Google Account on Family Link to see if this new child's account might be authorized. While they theorize this would work with a new account, it does not address any existing Supervised Google Accounts. For my end, I'm pushing them to address the existing accounts (as I have no interest in two accounts for one child and have an established set of guardrails for my youngster). I would be curious to know if adding a new Supervised Account to the Family Link today will actually work.
I hope this helps clear some things up. Fitbit is definitely going through some transitional challenges. Unfortunately, it seems these challenges have been going on for over six months now.

What in the actual eff. I bought this Ace 3 with the intent on managing my kids activity. This watch is now nerfed and I can't send activities or monitor my kids anymore.. also great they made the cutoff right before Christmas when we opened the **ahem** thing. I'm returning this crap after having to do so much work to even get it to sync with a Google account... I had to get an OLD phone and set that up for my son, now it means nothing but an added step to do account because of Family Link. Eff this nonsense.
