I've just received my replacement fitbit blaze do i have to cancel my old account to start a new one.

Keep same account. Choose set up a device and follow the prompts. I forget exact wording but something like asks which device you are setting up, and if it is one already registered it asks if this is a replacement device.
That will keep all your historical data, and add right on to it.
