Any chance the tile is hidden?

 

When tiles are hidden, there's a 'show hidden' button displayed at the bottom of the screen in the app on the dashboard. Tap the button to display all hidden tiles.

I'm curious if your referring to the tile as @SunsetRunner mentioned, or is it case of the add exercises not showing up.
