Walkmate Trial enrollment

Good morning

I tried to finish my registration for the Walkmate trial, but the application crashed. I did not know how to get the enrollment form again. I also can't find the Google Health card labeled "Walkmate" in the app.

May anyone help me fill out the form and join the trial?
