I set up my Blaze with an old phone, it broke and now I have a new phone. I cannot figure out how to set up my new phone with my Blaze. Everything I have tried so far just says my Blaze rejected it.

Your Blaze is alreasy setup to you account, there is no need to set it up again
Install the Fitbit app, log into your ditbit account, the app will then atteml to sync the tracker.
A fitbit tracker is never setup to a phone, but is attached to your fitbit account, then the app sets up the phone
