I would also add to what @Rich_Laue said that you can try to set it up as a new device and then turn on the notifications again to make sure it's all connected.
For setting it as a new device, go to the Account section, choose the option that says "Set up a new Fitbit device" and follow the on-screen instructions. Then tap on the picture of your tracker on the dashboard, scroll down to "Notifications" and enable them and see if this makes any difference.

Hmmm... Okay, there's another thing that you can try @SunsetRunner.
If you go to the bluetooth settins on your phone, you will notice that all your bluetooth connected devices will show up there. Look for the Blaze and tap on the "i" inside a circle and then tap on "Forget this device". Once you've done this, go to your Fitbit app, tap on the tracker and go down to "Notifications" and here turn them on. It will ask you to set up the tracker so just tap on Ok and after that see if this works.
