I have a charge 5 that is under warranty. The battery holds a charge for about 5 seconds. I have tried for over a month to get it to work with all the suggestions. My warranty will run out next month and that is what I am afraid of. If I have to run around in circles for much more time I am out my watch. There is NO form that I can find to start the replacement process. What do I have to do to get this going? PLEASE HELP......SOMEBODY.

Have you had a chance to contact Support (<-- click link) about the issues with your Charge 5? This is how you start the replacement process.
Complete the prompts on the Support page until you get to the option to chat or call. You might have better response if you call.
Although it shouldn't be this way, you will need to be your own advocate and followup with Support until you get confirmation that your replacement has been shipped. Make sure to keep track of your original case number.
