My sleep score records in the actual Fitbit app but it is not showing up on the device for the last few months. I refresh the app over 20 times a day and it still will not show up on the device.

This is a current bug that other users have reported. We are hoping that the issue will be resolved in a future update.
The good news is that although you aren't seeing your sleep score on your device, the fact that you see your score in the app means that your data successfully was uploaded to your account on the Fitbit servers. So there's no loss of your sleep data.
