heart rate assessment app not working properly

The heart rate assessment says it is installed. I followed all the steps previously outlined in other similar topic questions. When I click on the "view results" it doesn't ask any questions. When I click on the "start" from the watch window the notification for the app shows on the phone, but when I tap it nothing happens. I have logged out of fitbit. Uninstalled the app. Rebooted the phone. Reinstalled the app and still nothing.
