I got my dad a Charge 6 and he was not getting phone call notifications in real time. All he got was missed call notifications AFTER the call (if he didn't answer the call). I tried everything imaginable and could not get it to work.
Then I noticed he had an old Fitbit Charge 2, which he hadn't used in years (851 days according to the app), still connected to his account in the app. I was about to delete the Charge 2 from the account, but on a whim I decided to check if it had call notifications turned on. They were off, so I turned them on. And as soon as I did, he was receiving instant call notifications. I assume deleting the Charge 2 may have also solved the problem but I'm not going to test that theory since it is working.
This seems like a major bug and I'm sure it has driven some people crazy. Just posting this in case someone else is having the same problem and might find this solution helpful.

Am I understanding you correctly? Are you saying that not getting call notifications when call notifications is turned off is a major bug? So Fitbit should ignore that setting?
Never mind. I guess I misread; thought you were talking about the same tracker. Sorry.

Hello @sawyer8
I believe you are correct that the notification issue was due to your dad's Charge 2 still connected to his Fitbit account. Apparently, the Charge 2 settings overrode the Charge 6 settings.
Just an FYI: With the newer Fitbit devices, a user can have only 1 device connected to the Fitbit app.
Glad you got it figured out and your dad is getting his notifications on his Charge 6.
