I would like to use Spotify and Alexa/voice.  Do I only qualify for that with a premium membership?

No, you don't need Fitbit Premium,.rather you need Spotify Premium (or whatever it is called).

Alexa is not needed either.

 

Bear in mind that even if you have Spotify paid plan you can't store your music in your Fitbit smartwatch.
