do you have to have premium services to get skin temperature and blood oxygen levels..

No. I do not have premium. I am only able to get blood oxygen and skin temperature from my fitbit App on my phone, not from my fitbit.
I downloaded the clock face with the O2 sensor. I'm not sure if that makes a difference.

Adding to what @Rose2016 mentioned. Take into consideration that the Health Metrics tile is available with Fitbit Charge 4, Fitbit Inspire 2, Fitbit Luxe, Fitbit Sense, Fitbit Versa 2, and Fitbit Versa 3. To see historical data and your personal range for each metric, upgrade to Fitbit Premium.
For other devices, a Fitbit Premium subscription is required to see health metrics data. Learn more about it here.
