I subscribe to Fitbit Premium. I started in March with the free trial and have paid the monthly fee ever since. Each month when it renews I am no longer a premium member. In the past, I found some way to activate it but I cannot remember what I did. Does anyone know why this is happening and how I can restore the premium subscription?

Since the Premium subscription is auto-renewing each month but it doesn't activate it please try restoring your purchases by doing the following:
