Frustratingly, for the past week my ionic has not accepted my. It doesn't lock me out like it does with incorrect passcode entries, just simply doesn't accept it. I have reset my device a number of times and still have the same issue.
Also, since the latest update I've notice my heart rate isnt being recorded properly. There could be multiple hours through the day and my heart rate is ZERO. I have tightened and loosened the strap and reset the device.
I did have issues with the sleep tracking, but this seems to have sorted itself out a few days after a reset, but still doesn't seem to be as accurate as it used to be.
I got my ionic in March 2019 and it seems as if this the natural course for fitbit devices as I had multiple issues with my charge 2 after several months and had 2 replacements.
Starting to lose hope in fitbit devices a little.

I appreciate the troubleshooting tried and the details mentioned. In this case, I would like you to perform a factory reset and check if your Ionic starts tracking your heart rate and you're unable to enter the pass code correctly.
