How to get a quarterly step counts report from Fitbit?

My employer offers an incentive program based on quarterly step counts or activity time.  If I can achieve 600,000 steps or 1080 minutes of activity every 3 months I get a reward.  I manage to hit those goals but the problem is I can't find a way to document it in a way that meets their criteria: 

• All documentation must contain your name, the service date(s) or period, the service provider name, and service description. 

Is there a way to get a report from the app with my name and dates?

The program is called "My Wellness Rewards" and it is run by Wageworks

Thank you for your inquiry. Currently this isn't available but you can share your idea in our Feature Suggestions board. Search for the idea first to see if one already exists so you can add your vote. The more votes an idea gets the more likely the developers will try to implement it. 

You can export your monthly, weekly and daily data and also your all time historical data by following these instructions.
