Can't purchase a Protection Plan for my Versa 2

I bought a versa 2 in late November and was trying to buy a protection plan, but it didn't get processed properly. I tried to correct it but the form wouldn't let me enter the right credit card expiry date. 

 

The order # is A9429QNKQ. I attempted to make the payment in late November.
