I have bought a new Versa and the heart rate graph shows "No measurements" .
I have tried the solutions of
1) Logout
2) clear cache
3) Force stop the app
Still it doesn't show.
And my phone is one plus 3 which is a compatible device. I need a resolution to this urgently, i am planning to return the product, if i don't get any resolution, Its really disappointing that fitbit cant get a graph working.

Good, you are getting your HR graphs! (for what they are worth, don't read too much into them unless you are below 120bpm or so, there is a HR quality issue for many ppl).
You have manually selected an exercise activity on the watch, and after done closed with pressing on 'done' -- AND you have a confirmed sync with the mobile app afterwards (check last sync time in the mobile app), and when you go in the exercise tile it is not there? Please confirm.
Syncing problems are unfortunately common. Make 100% sure that you have a valid sync (it says the last sync time in the app on the versa page). If the sync time is before your exercise, then try to sync first. If it is after and you don't see the exercise in your your completed exercise list, then let the community here know!

Are you getting heart rate recorded on your Versa? If you are and not on the app I thjnk it is your phone that is the problem. I had a look on the Supported Devices List and there is only one Oneplus phone supported and unfortunately it is not your phone (it is the One not the Three), so I would guess that is the reason. Sorry I cant be more help.

I spoke to the customer care and Now the heart rate is working. However Now i have a new issue.
I have done an activity and its not showing up on the app. Repeated syncing problems , it looks like i may have received a defective device. Very disappointing.

Yes as you said syncing problems are a common issue on the Fitbit. The solution is to sync before activity and make sure everything connects.
1) kill the app
2) restart Bluetooth
3) open Fitbit app and sync.
This is what I have found works .

@prady32 I'm on OP3T. That's what helped bring my Versa back and I haven't had bigger issues ever since:
Very rarely I have to toggle bluetooth on and off now or shut down Versa. For whatever reason, Versa repaired itself when I forced the sync through the Fitbit mobile app. Do not ask me how.

So, I used to get a beautiful chart for my heart rates and now, all of a sudden, my heart rate chart is missing and is replaced with the message of "no measurements". I've tried every solution there is and nothing has helped. Any suggestions on how to get my heart rate chart back on the Fitbit app on my phone? Thank you!
