Assuming you have downloaded the app on your new phone (and logged in to your account) then:

Are you saying you can't install the Fitbit app on your new phone? If so, then I don't see a way for you to continue to use your fitbit.

Load the Fitbit app onto the phone @SunsetRunner
Log into your Fitbit account on the new phone.
A user does not connect a Fitbit tracker to a phone but the tracker connects to their Fitbit account.
Log into your account, make sure your logging into the correct account and not creating a new account.
Your Fitbit app will see the tracker and show that it is connected to your account.
Now follow the steps to unlock that @SteveH. pointed to.
These steps will work unless you removed the tracker from your Fitbit account. If you did remove the tracker let us know.
