I authorized Fitbit for my home computer, iPhone and Vitality Portal. However, under My Applications, I noticed this morning that Fitbit for Windows Phone was listed, and I do not own a Windows phone nor do I think I accidentally requested it. I revoked access, and then lo and behold it reappeared a few minutes later. Is there a legitimate reason why Fitbit for Windows phone should remain authorized? I'm mildly concerned that someone has hacked my account.

Hello @Microbios, its nice to see you here in our Community. I wonder if you are syncing with a computer using the windows app?
Even if you are not syncing with a Windows computer but if you sync somewhere else, specifically with the Windows app is possible that your dashboard is recognizing it as mobile phone.
I don't believed that your account has been hacked, however if you are not using any Windows device or the Windows app revoke the access from your application settings again and change your password using the following link.
Doing this you can be sure that no one has access to your account. If the issue persist you can approach to our Support Team so they can review your device history with you to investigate further.
Hope this do the work. Keep me posted if you need more help.
