I have worn all week and has not detected sleep and it is not reading my heart rate which is the whole reason I wear my fitbit. I have switched arms, switched placement, wiped off device and still nothing.

Welcome to the Fitbit community, @freitjas!
Sad to hear that you're having issues with your Versa 4. To improve sleep tracking with your device, I'd suggest setting up a sleep schedule and eventually change the setting of sleep sensitivity to better adapt your device to your sleeping habits. To do so please follow the instrucions provided here:
How do I track my sleep with my Fitbit device?
Concerning your issue with heart rate tracking, please have a closer look here:
What factors can affect my heart-rate reading on my Fitbit device?
