Is there a way to connect an ios App directly to the hardware for monitor real time heart rate ?

No; but keep in mind as long as your bluetooth and App are active on your iPhone or iPad you can monitor HR in realtime - I just go to settings>general>auto-lock and temporarily set to "never" when monitoring a workout.
