If you swap phones then you just need to download the fitbit app from the app store and logon using the email and password you used on your last phone. 

 

If you still have your old phone then it worth deleting the fitbit app and removing any connection to the fitbit in its bluetooth connections first.

 

Note though that your new phone isn't on fitbit's list of supported phones at www.fitbit.com/devices so there may be problems.
