Exercise Icon Not Showing

My exercise functionality was working until several days back when the icon disappeared.   The "Exercise" verbiage is still there and when I click on the empty spot it tries to start but returns to the same screen.

Failed to mention in previous post that have tried restarted several time with no luck.
