I have confirmed that heartbeat is turned on, repositioned it on my arm, cleaned the wristband, restarted the Fitbit; every troubleshooting tip I can find. Nothing has fixed the problem.

As you seem to have tried all the normal troubleshooting steps it may be a hardware issue. I have flagged your post for the attention of a moderator who can put you in touch with customer support. If they can't fix it then they can check your warranty status and advise on your options.
