I recently experienced a six hour period of high heart rate, 3 1/2 hours in peak. If my sense alerted me it was subtlety as I was unaware until it was over. Could a different form of alarm be possible, with sound or repeated vibrations? Does not make sense having it if it does not alert you.

Welcome to the Fitbit Community, @cpas605.
I'm sorry to hear about the difficulties you experienced with your Fitbit Sense. Thank you for the information provided.
There is a suggestion called "Audible alarms and notifications on Sense and Versa 3". You are welcome to vote it and if there is any update it should be communicated as soon as possible.
To make sure your heart-rate zone notifications are turned on, please visit this article, open the menu "What are heart-rate notifications? > How do I turn off heart-rate notifications?" and follow the instructions.
Have a nice day.
