issues in receiving the replacement package.

Hello fitbit support, 

I'm facing some issues in receiving the replacement package. 
As you know in the replacement form last name is a mandatory field and doesn't accept special characters either so I had to enter something(Sakshi karn) where as in my kyc documents is just (Sakshi) which is causing the name mismatch and I'm unable to receive my delivery. 
Can you please help with with that?
