I’ve just got a new iPhone 6s. Silly me, I removed my surge from the app on my old phone. How do I re install it on my new phone?

Thank you for your recommendations. Turns out the problem was that I still had the Bluetooth on on my old phone. So that was interfering. Once I switched that off my surge paired straight away with my new phone.
I am having another problem though. My surge drained from full charge in 12hrs the other day. I went through a few settings and switched off all day sync. Now it’s better but still only two days and the battery is down.
The surge is a brand new warranty replacement. But only for the strap coming off.
