How do I set my display on Surge to match the picture above?

I think that you didn't add the picture? You can edit your Surge Clock faces on the Fitbit App. Log in into your Fitbit Account > Surge > Clock Face. Then just sync your Surge! Is this what you were looking for?  Is your Surge a new tracker? Please add more details to your post to give you a more proper answer.
