My points have not counted all year for my vitality app. Everything is linked and I just revoked my access, but can't figure out how to give access again. Help me!

If Humana Vitality is the company, here is a chart of connected products and Fitbit is on it. The problem is I a search brought up several possible vitality companies. One is in New Zealand.
Check this Document to connect that is if I have the right company, a search of Google found this info.
