Unable to submit intraday request form

I have tried on 3 browsers and disabled ad blockers but the submit button on the form is blocked.

Please could someone advise?

I am having the same issue

So frustrating! Perhaps the form is not working. I have tried to go through customer support who promised to advised but I have not received their response yet. I will share here if I get a solution.

I wondered if you could provide an update on the issue with this form? Our app launch is being delayed as a result of this. Would it be possible to submit the research compliance form in the meanwhile so that we can reduce delays later?

@Gordon-C @ImperialPM I wanted to bump this item up.  I went in today to try and submit the request form and I am still unable to do so.
