So frustrating! Perhaps the form is not working. I have tried to go through customer support who promised to advised but I have not received their response yet. I will share here if I get a solution.

I wondered if you could provide an update on the issue with this form? Our app launch is being delayed as a result of this. Would it be possible to submit the research compliance form in the meanwhile so that we can reduce delays later?
