Blaze failed to sync and now won't pair

Please help - my Blaze stopped syncing to iPhone 6.  I tried to reset bluetooth, and to reset the device.  Neither worked, so I un-paired the device, and now it will not pair.  I get to the step of entering the 4-digit number from the device, and it fails.  Have tried 3x with no success.
