Fitbit app gone from Ipad following update

I have had my Fitbit Alta for around 18 months and have had it as an app on my Ipad using it for steps and also as my alarm in the morning. Suddenly it has disappeared from my Ipad and I am unable to see any info or set my alarm.

After an update around 2 weeks ago I was able to get it going again but 2 days ago it has gone and I cannot find the little star shaped group of blue dots  app anywhere
