After connecting my pixel watch 3 to a new phone all of my historical data for exercise title is missing. I have tried the usual uninstall, clear cache and re-install steps but still missing my data for past 5 years.

There is a known issue on the Fitbit side that affects our ability to see historical data. This affect users of multiple Fitbit devices, including the Pixel watch.
I've seen recent Moderator posts acknowledging the issue and that the team is working on fixing the problems. They also mentioned that an app update will be rolling out in the next few days. Keep checking the Play Store for the app update and install it when it's available.
I'll check back when I see confirmation that the update has started rolling out to see if it fixed the problem you're experiencing.
