Step count disappeared from my dashboard. How do I get them back.

If just the tile has disappeared then in the upper left corner is a circle with squares in it.  Click on that then add the tile back in.

I have a Blaze and my dashboard on my PC only showing floor and all other informations disappeared.  Can someone help?
