I have had the blaze for a year (it was replaced in March because it wouldn't turn on and the band was replaced over the summer) Since it was replaced it randomly un-syncs from my phone. I have in-installed and re-installed the app, disconnected the Bluetooth, forgotten and re-added the blaze, re-set the blaze. I am starting to get annoyed since it happens a few times a week at least.

I'm very sorry to hear that you're having problems getting your Blaze to sync to your phone and thanks a lot for all that you've tried to get it to work. I'll be happy to help you out.
The first thing I'd like to ask is if you have made sure that the phone that you're using is compatible for syncing. If you're not sure about this, check the list of compatible devices on this article.
If your phone is not compatible, this could be the reason you're having problems. If it is compatible, we can take a deeper look and see what else could be going on. In either case, let me know so that I can provide further assistance.
