I have a charge 5 and when I go to take ECG it says to open the assessment app. I do that and it says to download the ECG app which I have... but then my watch still won't recognise it and I get sent round the loop again! ? I have premium. Am I missing something?

I do ECGs regularly and have no idea what you're talking about. I don't have to open any separate app to take one.
