This is the second time I used my new Iconic to counts laps during my long distant swim this morning in the pool only to discover that it stopped counting and went back to the clock screen. It has also done the same while biking and also looses GPS signal occasional during a run.

@Roadrunner6820 I hope you're doing well! Thanks for reporting the situation you are experiencing with exercise app shutting down on Ionic.
By any chance, did you double tapped the Ionic's screen before the screen went back to the clock face?
I'd recommend performing a restart on Ionic. This could help you out. Also, how often is this happening to you? Have you noticed this happening after doing something in particular (tapping the screen, receiving notifications, etc.)?
Regarding GPS, I'd recommend checking this helpful article to get some information as to why this could be happening.

I happed one day last week during a swim. On Sunday, 7/15, it did it about 6 minutes into the swim. On 7.16 it did it at 54:21 into the swim, and today 16:28 into the swim. All of my swims have been 3000yd continuous swim taking about 1 hour, thus no tapping of the screen. I can see when it malfunctioned by looking at the Activity History. Moreover, I have the notifications turned to off on my Fitbit..

Did you tried restarting Ionic? This could help you out with this.
Also, have you noticed if this is happening with other types of activities? or just with swimming?

I did the soft reset of turning the watch off on 2 separate occasions and it did not work. I then did the factory reset last week on Wednesday, 7-18-18, and so far it has been accurate using the GPS for running three times and biking twice. For swimming it has been accurate on lap counting for the 2 times I swam 1200yds. Haven't used it on a distance swim nor in open water yet. Hope this continues.

@Roadrunner6820 Thanks for your reply and for providing those details about the exercises you are performing with Ionic. I'm very glad to read that they are being accurate for you now. If it doesn't remain like that, please let me know.
