Versa heart rate not working

My heart rate tracking is completely off. Whenever I'm doing exercise it turns off completely for about a third of the time, if I'm doing an intense workout it thinks my heart rate is 90 despite the fact that my heart is racing and probably more like 160, and if I'm going for a casual stroll and my heart rate is about 85 when I test it myself, it thinks I'm at 130. I've adjusted the placement many times, adjusted the tightness, nothing helps. It just doesn't work.

Thanks for troubleshooting this by yourself. If you are having issues with your heart rate information being recorded, I recommend taking a look at this post, where you will be able to see some things that might be affecting this reading and some tips to improve it.
