After every 4-5 days the app notifications stop showing up on my fitbit sense device. I had to factory reset the device the last time, however it's not feasible to do that every 4-5 days. I tried restarting the phone as well as the fitbit device but all in futile attempts to get the notifications to be working.

First, welcome to the Fitbit community.
Just want to clarify, are you not getting any notifications at all (It says "You're all caught up!") or you have notifications but no vibrating alerts?
If it's the first, I would try deleting Sense from the Fitbit app and adding again. You don't lose information nor does it factory reset.
If it's the second, try turning on always vibrate:

A factory reset is never the answer. It is helpful to know what OS the phone in question runs,
First check the easy, is sleep and DND disabled?
Then turn off notifications and set them up, this is the fastest and easiest way to reset permissions.
On Android, disable power management for the fitbit app.
