Screen turned yellow and has pixels and at times lines. I tried resetting and took it off the app and restored on the app but the screen remains unchanged. Any advice?

Once moisture enters the screen it will change, the seals for the screen are not strong enough, and over time it fails. When my Ionic failed the screen changed to many different colors. If under warranty they might replace it, try that route first.

In that case, there are a few others things to try, put it in a bowl of rice/drying silica/cat liter, you might be able to draw out the moisture from the device. That did work for my Ionic, and it did work after, but there was still damage to the screen. Another solution is to get a replacement screen from a repair place and either have them replaced it or you can do it yourself.
