Battery Drain & Continuous Vibration

My Charge 5 battery was lasting a week between each charge, but last week it suddenly drained the entire battery in a couple hours and it won't hold a charge for even a single day anymore.

Last night it says I did two intense elliptical workouts while I was asleep.

Today it is starting to vibrate continuously for no reason, resetting stops it for a little, but then it gets right back to buzzing.
