I noticed the same thing today. There is no plus (+) sign next to the exercise icon on the "Today" screen. I recently upgraded to iOS 13.1.1 and Fitbit version 3.7 (872).

I did find two workarounds to get to that screen:

1) On the main iOS home screen, hold your finger on the Fitbit icon. A menu will pop up with six options (track exercise, Log weight, Log food, Log water, Rearrange apps and Share Fitbit). I never noticed this feature before (not sure when it was introduced but it works.) Just click "Track Exercise" and it will bring up the menu GPS menu to manually start a workout. This is actually easier then going into the app and clicking the old plus (+) 🙂

2) Click on the Exercise tab (where the plus + used to be)

-- Click on stop watch on top right of screen

-- Click TRACK on top of screen to bring up screen to manually start workout.
