Surge disconnecting from bluetooth

I have had a Fitbit surge for just over 1 year and have LOVED it. In the past month, all of a sudden it continually disconnects from Bluetooth on my iPhone. I have forgotten device, shut both devices off and rebooted multiple times. Please help. I primarily upgraded to the surge from my original Fitbit for the phone and text notifications and now I'm not getting them.
